The company also owns and operates 3 Intermediate Care Facilities (ICFs) as well as providing services in Licensed Residential homes. Midwest Community Services is a Medicaid provider which offers a variety of day program services including Adult Day Support, Vocational Habilitation and Non-Medical Transportation. We are devoted to providing innovative programs, services, and support to adults with Intellectual and Developmental Disabilities. We have several community centered locations throughout Ohio and continue to grow and serve.
Community Health Network, LTD is a CHAP(Community Health Accreditation Program) certified Home Health Care Provider that is committed to providing quality nursing and therapy (PT, OT, and ST) supports for adults and children with Mental Health or Intellectual and Developmental Disabilities. The agency is also Medicaid and Medicare certified.

Midwest Health Services was founded in 2000 by Joseph B. and Kristine Knetzer to provide superior supports to persons with intellectual and developmental disabilities. Today Midwest Health operates intermediate care facilities and provides needed supports to people in congregate settings and in their own homes. We are committed to listening to those we serve every day in order to maintain the highest level of care.
