Our Mission

Community Health Network is committed to providing quality nursing supports and therapies (PT, OT, ST) for individuals with Mental Health, Intellectual and Developmental Disabilities. Our commitment to this is evident due to the personalized approach we take to hiring, training, and placing new staff. We know how important it is that those we serve have someone that is going to advocate for them, and no corners are cut when looking for potential caregivers. Through this we make sure our focus is evident, which is to enable the pursuit of hopes and dreams for those we serve through total community inclusion.

CHN exists in and is certified in the State of Ohio to provide the following supports:

  • Private Duty Nursing
  • State Plan Nursing
  • TDD/IO Waiver Nursing
  • Medicare Skilled Nursing Services
  • Provide PT, OT, and Speech Therapy
  • Coordination of medical care
  • Personal contact with physicians and other health care providers, including attendance at medical appointments
  • Health assessment of individuals
  • Injury assessment of individuals and employees
  • Nursing care in ICF-IDD environments
  • Individual specific medical training for employees
  • DODD Medication Administration Training for Oral meds, G-Tube medications and feedings, and Insulin administration for unlicensed personnel
  • Nurse medication administration
  • Nurse Delegation
  • American Heart Association CPR and First Aid training

Experienced and Caring Staff

CHN is extremely fortunate to have assembled the team of nurses and therapists that we have. The IDD field requires compassion, empathy and patience, and our experienced staff of Licensed Practical Nurses (LPN), Registered Nurses (RN), and Therapists (PT, PTA, OT, OTA, ST) are very familiar with those requirements. As nurses and therapists our staff are also true examples of caregivers, and are committed to providing you or your loved one with the highest level of care.

Care Coordination

When it comes to making sure that everyone is informed of any changes, updated orders, etc. Community Health Network knows how important it is to keep you involved! From making sure the Service and Support Administrator (SSA) is aware of a change for the ISP, helping you coordinate a visit with the Doctor, or ensuring that you can attend a therapy visit even though you work odd hours we’re going to help make sure everyone is on the up and up.
